Communication > Build Custom Forms
This section gives you an overview of existing forms (once created) and the ability to add, edit, duplicate and delete forms as well as view responses.
Building Forms
- Click on Add Forms button to access the form builder
- Enter a title and (optional) description
- Chose if you want respondents to sign the form when submitting
- Add your questions by clicking on or dragging the question/field types from the options in the menu:
- Checkbox Group - ability to select one or more pre-determined answers
- Date Field - allows the respondent to select a date
- File Upload - allows the respondent to upload/attach a file or photo to the submission
- Header - add a fixed-text heading (added by the form builder)
- Number - restricts the field response to only a number
- Paragraph - Fixed text/instructions in your form (added by form builder)
- Radio Group - the ability to select only one option from a selection of pre-determined answers
- Select - ability to select one or more pre-determined answers
- Text Field - allows the respondent to enter a single line of text
- Text Area - allows the respondent to enter a paragraph of text
Editing and Copying Questions

Hover over any question to view the options in the top right corner of the field. Click on the pencil icon (middle) to edit the question details and options.
These options vary depending on the field/question type selected and usually include options for 'Label' which is where you would enter the question, access (who can view/respond to a particular question) or other various display settings. For multiple-choice answers, this is also where you can enter the predefined responses.
You can also change the order of questions by dragging them up or down.
Saving & Publishing Forms
Forms can be saved as a draft if you aren't ready for people to start submitting responses or Publish the form if it is ready to go live.
Submitting Form Responses
Staff can submit from responses from the app (under Forms in the Home menu).
Employees are unable to edit their form responses after submitting, however, they can submit multiple sets of answers.
